Patient Safety is now recognized as a primary goal for modern healthcare. Harm that results from the delivery of healthcare interventions is both distressing and expensive. As part of any risk management process, an organization needs an effective investigation process. The scale of an adverse event will dictate the scope and complexity of any investigation. However, the fundamental principles of investigation and the goal of the process remain the same irrespective of the nature of the event. This workshop outlines the nature of the investigation process, explores the conduct of investigations and examines the application of different theoretical models of accident causation. The role of the investigation process in organizational learning will be examined. Delegates will experience aspects of the investigation process through the use of case studies.
